When you have completed the form you can send it to us straight away using the 'Submit by Email' button, this will open your e-mail application and attach the filled out form to the e-mail. If you wish to send the completed form using a web based e-mail system such as Hotmail, Yahoo Mail or AOL save the from when you have finished completing the fields and send it as an attachment to bridging@cmls.co.uk. You can print a copy for your records and save the completed form on to your computer as a record for your files. If you don't want to submit the DIP by e-mail you can print the form using the 'Print Form' button and send it by Fax or through the post.
